Sick Log Documents
Below is an example Sick Log document that you can download and use in your establishment, as well as a recommended “Return-to-Work Recommendations" document.
Use the Sick Log to enter an employees name and the date they are excluded from work due to the signs and symptoms of pandemic coronavirus or foodborne illness using EmergiProtect. Use the Return-to-Work Recommendations as a guide to determine when you may allow employees to return to work.
